FLCH and Mosaic Health exploring merger to expand community care

Finger Lakes Community Health announced plans today to explore the possibility of Rochester-based Mosaic Health becoming part of its organization. The move aims to combine resources, increase access to healthcare, and enhance the delivery of services across Upstate New York communities.

Leaders from both organizations described the merger as a significant step toward addressing evolving healthcare needs. FLCH CEO Mary Zelazny said the potential partnership would enable the organizations to build resilience and better advocate for the needs of their patients. “Mosaic Health joining FLCH would strengthen our ability to serve diverse communities, enhance health outcomes, and give us a more influential voice in advocating for community health as our patient needs evolve,” Zelazny stated.


Mosaic Health President and CEO Michael Leary echoed these sentiments, highlighting the shared mission of the two organizations. “FLCH understands the patients we serve, and together, we would be stronger and offer more services. Our patients would not see any changes in their experiences other than expanded availability of care,” Leary said.

Currently, the two Community Health Centers serve nearly 43,000 patients across Upstate New York. As part of a nationwide network of health centers, they play a key role in providing medical, dental, behavioral health, and care management services to underserved populations. In New York alone, 1 in 8 residents utilizes a Community Health Center for care.


Pending regulatory approvals, board decisions, and due diligence, Mosaic Health could officially merge with FLCH by the end of 2025. Details about the future name of the combined entity remain undecided.
