Investigators allege Willoughby, who was acting as a child day care provider, caused a serious injury to an infant in her care. She was charged with reckless assault of a child by a day care provider, a felony, along with third-degree assault and endangering the welfare of a child, both misdemeanors.

Willoughby was issued an appearance ticket and is scheduled to answer the charges in Benton Town Court at a later date.

The investigation involved the Yates County Sheriff’s Office, Child Protective Services through the county Department of Social Services, and the New York State Office of Child and Family Services.

Authorities have not released additional details about the child’s condition.
