The Livingston County Center for Nursing and Rehabilitation (CNR) has received a four-star rating in the 2024 quality star rating report issued by the Centers for Medicare & Medicaid Services (CMS).
The recognition, which marks the second-highest distinction in the CMS rating system, places the CNR among the “above average” nursing homes across New York and the nation. Deputy Director of Long-Term Care Eric Townsend credited the dedicated staff and a commitment to resident-focused care for the achievement.

“We are so proud of everything we do here, and I think I speak for everyone at the CNR when I say it’s incredible to be recognized for our work,” Townsend said. “This recognition motivates us to continue improving and delivering the highest standard of care to our community.”
The CMS star rating system evaluates facilities on health inspections, quality measures, and staffing levels to provide families and caregivers with a clear comparison of nursing homes nationwide. Rankings are featured on Medicare’s Care Compare website, offering transparency and guidance to those seeking long-term care options.
The Livingston County CNR provides 24-hour skilled nursing services for its 266-bed facility, ranging from short-term rehabilitative care to long-term residential care. The center prides itself on tailoring its services to meet individual needs, fostering a homelike environment with activities involving family, friends, volunteers, and even pets.
In addition to traditional care, the facility offers a comprehensive array of modern healthcare services, including audiology, occupational therapy, speech therapy, dentistry, podiatry, bariatric care, and more. This diverse range of services cements the CNR’s reputation as a leading option for personalized, high-quality nursing care in the region.
