There are many reasons someone may want to change their legal name, like marriage or divorce.

If you need to change your legal name, you must let the Social Security Administration know as soon as possible.

To complete this, you must file Form SS-5 which is an application for a SSN card.

You may not send copies of these documents, they need to be original.

You can mail them or physically drop them off at your local SSA office.

Citizens must prove their citizenship as well.


For legal name changes you must show and original marriage certificate, divorce decree, certificate of naturalization, or court ordered name change.

For proof of identity you must show a U.S. passport, driver’s license, or state issued document proving your identity.

Show your citizenship with a U.S. birth certificate, U.S. Consular Report of Birth Abroad, U.S. passport, or certificates of naturalization or citizenship.
