The program built by Finger Lakes Health is an effort to improve patient outcomes. Best yet, it’s a program that’s available to all people who have experienced an acute care stay at a hospital in the 30 days leading up to admission to the program. It helps patients transition from life inside a hospital to whatever comes next – whether that’s home – or additional long-term care.
