Only a small fraction of home care agencies nationwide earn the top federal quality rating, and Rochester Regional Health Home Care is now one of them.
Rochester Regional Health announced Monday that RRH Home Care received a 5-Star Quality of Patient Care rating from the Centers for Medicare & Medicaid Services Home Health Star Ratings Program. The designation makes it the only home care provider in the region to earn the highest possible score.
What the rating means
CMS evaluates more than 12,000 home health agencies across the country on patient outcomes, experience, and performance. Just 429 agencies — about 3.5% nationwide — currently hold a 5-star Quality of Patient Care rating.
RRH Home Care’s score reflects strong performance across measures such as timely starts of care, improved patient mobility, better daily functioning, medication management, and fewer preventable hospitalizations. The rating also accounts for behind-the-scenes work, including accurate assessments, reporting, and ongoing clinical improvement.

“This milestone belongs to every member of our Home Care team,” said Sara Falk, vice president of Home Care for Rochester Regional Health. “It takes seamless collaboration across clinicians, caregivers, operational staff, and our quality team to achieve this level of excellence.”
Falk said the recognition reinforces trust with patients and families and reflects care delivered with “dignity, compassion, and respect in every interaction.”
Serving patients across the region
RRH Home Care operates as a Certified Home Health Agency, serving patients in their homes across Livingston, Monroe, Ontario, Seneca, Wayne, and Yates counties. The program relies on interdisciplinary teams that support patients while helping them remain at home.
Rochester Regional Health said the 5-star designation supports its broader “Care Like Family” commitment and signals that patients can expect care that meets the highest national standards.
