Police say a Penn Yan woman was arrested following a forgery investigation.
According to a news release, the Canandaigua Police Department arrested Sarah Nussbaumer, 40, of Penn Yan for forgery.
It is alleged that Nussbaumer entered a local hospital where she provided a fake name, date of birth, and address. She signed a patient care agreement which included her acknowledgement of financial responsibility for services rendered.
Nussbaumer was taken to the Ontario County Jail to await arraignment.
